Job Description
The Care Promoter Licensed Practical Nurse (LPN) - Lead is a healthcare and leadership-focused position working directly with patients and their families, doctors and other medical personnel in a dynamic and professional environment to provide the highest level of quality healthcare to all patients.

In addition to conducting all activities as outlined in the Job Description for Care Promoter 3 Licensed Practical Nurse, the Lead will perform the following duties:

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

  • Provides extraordinary customer service to all internal and external customers (including patients and other team members) at all times.
  • Manages and leads the Care Promoter Team, ensuring the completeness and accuracy of all tasks.
  • Monitors team performance, identifying any issues and provide Center manager with feedback.
  • Addresses / resolves any customer-service issues.
  • Ensures proper flow of patients/doctors from check-in area to patient exam rooms (“directing traffic”).
  • Assists in triaging of walk-ins and patient phone calls.
  • Ensures accuracy of medications provided to patients.
  • Ensures accurate indexing of all paperwork.
  • Monitors/ handles deliveries and invoices for medical products.
  • Monitors team member work schedules.
  • Monitors waiting time report.
  • Ensures rooms for patients are prepared with all medical tools.
  • Checks over patient schedules for transportation pickup.
  • Inspects pharmacy log and inventory.
  • Monitors physicians’ activities and assist with their needs.
  • Discharges patients in accordance with physicians’ orders.
  • Inspects crash cart on a weekly basis.
  • Coordinates sending out of lab specimens.
  • Acquires reports from hospitals and urgent cares for patients coming in to see physician.
  • Manages phone messages and call patients back.
  • Ability to travel to other offices in market for assistance/coverage.
  • Other duties as assigned and modified at manager’s discretion.

KNOWLEDGE, SKILLS & ABILITIES:

  • Knowledge of ChenMed Medical products, services, standards, policies and procedures.
  • Skill in operating phones, personal computer, software and other IT systems.
  • Skill in oral communication (English required).
  • Skill in taking patients’ vital signs, obtaining blood and other specimens for laboratory work.
  • Ability to communicate and work with doctors, patients and others in a professional and courteous manner.
  • Ability to pay close attention to detail and to ensure accuracy of reports and data.
  • Ability to manage multiple processes and lead a team of Care Promoters.
  • Ability to organize and prioritize. Ability to remain calm in stressful situations.
Additional Job Description

EDUCATION AND EXPERIENCE CRITERIA:

  • High school diploma or equivalent and graduation from a nationally accredited school for practical or vocational nursing required.
  • One (1) year of related clinical work experience required; three (3) years clinical experience is preferred. Experience working with geriatric patients is a plus. EMR system experience is preferred.
  • Current, active LPN license to practice in state of hire required. IV Therapy Certification if required by State Board of Nursing. CPR certification for Healthcare Providers required.
