For patients requiring off-site care from a trusted specialist, JenCare staff will coordinate the entire referral process.

Behavioral healthcare is an emerging field within the practice of high-quality, coordinated healthcare. It is when behavioral health specialists and medical providers work together to care for a patient. This care may address mental health and substance abuse conditions, health behaviors (including their contribution to chronic medical illnesses), life stressors and crises, stress-related physical symptoms, and ineffective patterns of healthcare utilization. Cardiology is a field of medicine that focuses on the heart and circulatory system, also known as the cardiovascular system. Cardiology includes the prevention, diagnosis and treatment of congenital heart defects, coronary artery disease and heart failure. JenCare cardiologists are experts in dealing with disorders of the heart and work onsite in many of our centers. At JenCare, our primary care physicians work closely with our cardiologist to coordinate your personal care plan and help keep your heart, arteries and blood vessels healthy. We specialize in helping our patients manage conditions like high blood pressure, high cholesterol, diabetes and other health issues that can lead to heart disease. Your JenCare PCP and cardiologist are a team and continuously communicate with each other about your heart health.
A case manager is a healthcare professional who helps patients and their families and/or caregivers get through tough times. A case manager can give advice and help patients who have disabilities, mental health issues or social needs find services they need to maintain optimal health and quality of life. He/she also works closely with doctors, social workers and other community service providers to ensure patient needs are met. As a JenCare patient, we can link you to Community Connections services, which include support from a case manager, if life’s challenges may be affecting how you live. Your case manager can help you link to community agencies for help securing food, shelter, in-home support and other needed services. He/she can also help you prepare for a hospital discharge and your aftercare needs, connect you to support groups and recommend activity programs that improve your health. Your case manager will help guide you along the way and follow up for ongoing support.
A hospitalist is a medical doctor who works full-time in a hospital. As a JenCare patient, you never have to worry about being alone in the hospital. JenCare has hospitalists onsite that oversee your care during your stay. This doctor is available to you at all times and keeps your primary care physician and specialists notified and up to date on your condition and coordinates all of your in-patient care until you go home.
An echocardiogram (or “echo” for short) is a sonogram of your heart. An echo is a test that uses ultrasound waves to take a picture of the heart and its other structures. An echo can provide your doctor with useful information, such as the size and shape of your heart. This test can also help your doctor measure how well your heart is working, check for fluid around the heart and locate defects or infections. At JenCare, we perform echocardiograms right in our centers, so you don’t have to travel to a diagnostic center for this important test.
This is a place where people get prescription and over-the-counter medications (also called OTC medications). As a JenCare patient, you can enjoy the convenience of getting the medicines you need right from your doctor during your visit. Getting your medicine while you’re here in our centers gives you and your doctor more time to talk about how and when to take it. You can also ask your doctor questions about your medicines. For you, that also means no waiting in long lines at the pharmacy or having to remember to get your refills in time. This branch of medicine focuses on mental health and emotional problems that can affect your behavior and quality of life. At JenCare, our Primary Care Physicians (PCP) understand the connection between mind and body health and can provide help for depression, anxiety and other common problems. Your PCP may also refer you to Community Connections whose professionals work closely with JenCare patients who require a more in-depth evaluation of their behavioral health needs and referral to helpful resources. Your PCP and case manager and/or social worker will always follow through with each other about your care, results, changing health and other needs.
This medical specialty uses radiation through tests like X-rays and ultrasound to diagnose and treat certain health problems and diseases. JenCare provides many different diagnostic services right in our centers so you don’t have to go to multiple places to get them done. Our services include EKGs to test your heart, lab work such as urinalysis, blood work, flu and strep tests, X-rays, ultrasounds and more. Your doctor will determine which diagnostic services you need and order them during your visits.
Skilled nursing is a level of care given when a patient needs full-time care from a nurse or other skilled nursing professional (SNP) to help the patient recover, especially after surgery, an injury or a hospital stay. This level of care may be required for short- or long-term needs. A skilled nursing professional can help manage, observe and evaluate your care outside of a medical office or hospital—usually in the home or a skilled nursing facility like a nursing home. Examples of skilled nursing care include IV injections, physical therapy and personal care like help with bathing, dressing and walking if you can no longer do them by yourself. Patients who have Medicare can get skilled nursing care under certain conditions.
